Rates and Insurance

Initial Assessment Sessions:

• $180 per 60-minute intake assessment session

• $150 per 55-minute counseling session 

Lindsey is an in-network provider for BCBS of Arizona, Aetna, Optum (not EAP), United Healthcare, UMR.

For clients who wish to use any other insurance benefits for counseling services, Lindsey can provide a ‘Superbill' that clients may submit to their insurance company for out-of-network reimbursement as their policy will allow.

Please call your insurance company directly to verify your specific policy benefits prior to your initial session.

Kaleidoscope Wellness PLLC is a private pay therapy and wellness practice.

Our practice offers a sliding-scale fee structure to help make quality therapy and counseling services accessible to everyone in the community.

There are a limited number of sliding-scale fee slots available, so please contact Lindsey to inquire about this option.

Life happens, and unexpected things come up. If you should need to cancel a scheduled appointment, please email Lindsey or leave a voicemail at least 24-business hours prior to your scheduled session. This allows Lindsey to offer that session time to another client who may also need to be seen.

If you do not provide Lindsey with adequate notice (24-business hours minimum), a cancellation fee of $65.00 will be applied to your account. Cancellation fees are not covered by insurance and are your responsibility.
